Providence Palliative Care MSW *Up to $5,000 Hiring Bonus* in San Pedro, California

Providence is calling a Palliative Care MSW (1.0 FTE, Day shift) to Little Company of Mary Hospital to work with the Providence TrinityCare Palliatve Care Team. $5,000 Hiring Bonus for eligible external candidates who meet all conditions for payment - this is in addition to the fantastic benefits and compensation package offered by Providence that begin on your first day of employment.

We are seeking Palliative Care MSW who will participate in the activities of the interdisciplinary Palliative Care Team. The Palliative Care MSW participates in the development and review of appropriate protocols, facilitates formal and informal meetings regarding patient and family communication and decision making processes that support appropriate referral and consistent care. The Palliative Care MSW engages in independent decision-making about psychosocial needs, provides psychosocial services to individuals and their families, and provides formal and informal meetings regarding patient and family communication and decision making. The PC MSW obtains a psychosocial history, including chart review and information gathered from the patient and family, and conducts psychosocial palliative care needs assessments. The PC MSW provides direct patient care as well as consultative services to assure the delivery of state-of- the-art palliative care. In addition to these clinical duties, the Palliative Care MSW is responsible for contributing social services work expertise to staff education, data collection, and quality improvement activities as they relate to inpatient palliative services.

Required qualifications:

  • Master's Degree in Social work or equivalent

  • 3 years of Inpatient, Outpatient, or home-based Social work

  • Demonstrated evidence of experience in palliative care settings, such as hospice, i.e., ability to:

  • a.Work independently as well as collaboratively on a small interdisciplinary team,

  • b.Provide and model interdisciplinary palliative assessment and management of physical, psychosocial and spiritual symptoms during aggressive treatment and end of life care.

Preferred qualifications:

  • Working knowledge of research techniques, data collection, and policy, program and curriculum development

  • Experience with quality improvement methodology

Providence Home and Community Care (HCC) is a service line within Providence with over 8,000 caregivers and 2,000 volunteers serving nearly 200,000 people a year, in their homes, in clinics and in a variety of congregate living ministries across our seven-state footprint.
