Providence Nursing Director Post Acute Care Services, San Pedro in Los Angeles, California

The Director of Post-Acute, Rehabilitation and Transitional Care Services is responsible for the overall direction of the services provided. The leader is responsible for the employment of qualified personnel, for the day- to- day functions of the facility in accordance with current, federal, state and local standards regulations and guidelines. The Administrator establishes, implements, and evaluates goals and objectives for services provided which meet and promote the standards of quality and contribute to the total organization and philosophy. The Director assures that staff and organization are current on local, state and national issues and trends.


  • Ensures program compliance with legal, regulatory and accreditation requirements.

  • Provides leadership in the development and implementation of organization objectives, standards of care, and policies and procedures affecting the programs of the organization.

  • Evaluates the program services and personnel using measurable outcomes and objectives. Responsible for development of goals based on annual evaluation.

  • Monitors operations for progress toward accomplishing operational and strategic plans.

  • Coordinates other program areas and senior management team.

  • Assures standards of ethical business and clinical practice are maintained.

  • Ensures adequate and appropriate staffing and supervision during all service hours.

  • Ensures staff development including orientation, in-service and continuing education to assure that staff stay current with clinical information and practice and the utilization of effective measures of competence and improvement.

  • Assures that supportive services are available to staff.

  • Assures that comprehensive services are provided to patients and families.

  • Maintains productivity standards as set by organization guidelines.

  • Participates in Senior Management committees at PLCMMCT.

  • Participates in the development of organization’s strategic plan and leads in its implementation.

  • Additional functions include:

  • Documentation / Information Management:

  • Establishes and maintains effective channels of communication including the integration of technology with communications.

  • Ensures appropriate data collection and regular reports are received by the governing body.

  • Leadership / Supervision:

  • Ensures that the organization’s structure and systems promote interdisciplinary care.

  • Ensures that adequate space, equipment and supplies are available.

  • Ensures collaboration with agencies and vendors for effective management of services.

  • Provides direction to staff in performance of their duties including admission, discharge, transfer and provision of services to patients.

  • Negotiates and signs contracts and agreements for provision of services by other agencies.

  • Assures enforcement of the agreements and delegates negotiation and implementation responsibility to the appropriate director.

  • Participates in orientation, education and development of team members as directed by supervisor.

  • Attends Operations Meeting, Care Council and other leadership forums as identified.

  • Some actions may require approval by the Chief Nursing Officer (i.e., recruitment for vacant positions, discharge).

  • Supervised by: Chief Nursing Officer.

  • Development:

  • Participates in appropriate continuing education as may be requested and/or required by immediate supervisor and/or governing body.

  • Ensures the organization’s commitment to maintaining confidentiality of patient information through development, maintenance and monitoring of related policies and procedures.

  • Accepts personal responsibility for other educational activities to enhance job-related skills and abilities.

  • Performance Improvement:

  • Actively participates in the Performance Improvement Program in both acute and post-acute setting.

  • Ensures all Core Measures, NPSG, Title XXII, Skilled Nursing standards, and other related specialty standards are integrated into Post-Acute Care Practice.

  • Identifies areas of improvement in services and utilizes the C.A.P. methodology to carry out and document these activities. Uses C.A.P.; Six Sigma tools and resources to advance and promote quality and safety in practice and patient care.

  • Budgetary:

  • Responsible for the final preparation of the budget for approval by the governing body.

  • Implements budgetary planning, monitoring and corrective action as needed to achieve financial stability.


Required experience/education for this position include:

  • Bachelor's Degree in Business Administration or healthcare related field

  • Master's Degree in Health care or related field

  • Upon hire: California Registered Nurse License (Vendor Managed)

  • Upon hire: National Provider BLS - American Heart Association (Vendor Managed)

  • 3 years of experience in health care management

  • Demonstrates ability to supervise and direct professional and administrative personnel.

  • Demonstrates ability to deal tactfully with the community

  • Has knowledge of corporate business management

  • Understands post-acute care services provided to patient/family/caregiver through an interdisciplinary group.
